This health providers and support services website covering the Horowhenua, Manawatu, Otaki and Tararua districts was commissioned and is fully funded by these four Primary Health Organisations.

This website is updated monthly.
 
The attached questionnaire requests the information we want to include on the site, but we can also include logos etc if you require.

You will be contacted each month to ensure that the site is always up to date so an e-mail address is important.  Please indicate if it is a private address so it will not show on the site but be kept for monthly update use only via a blind copy.
 
This site is modelled on a very successful site we have already set up for the Kapiti district which can be viewed on: www.kapitihealth.org.nz
