Important Patient Information:
•Plan your appointments ahead of time for your regular check ups.
•Tell us if you need more time than the usual 10-15 minute appointment, so it can be allocated to you e.g. Insurance Medical, Drivers Licence etc.
•Tell us if more than one member of the family is to be seen when you make your appointment.
•If you have several issues or problems that you wish to discuss please request a double appointment.  In the event that you do not have a double appointment you may be asked to come back another time to go over your other issues allowing us to deal with your most urgent issue first.
•Please report to reception on arrival the front desk can become very busy and we may not  always know you have arrived.
•If you have had a change of address or phone number please tell the receptionist on your arrival.

Fees:
•Payment on the day of the consultation would be appreciated and is expected. 
 
Accounts: 
•The practice does not operate a credit facility and does not therefore offer an account.  No one individual may owe more than $100 at any given time. Fees not paid within 30 days will attract an administration fee monthly until paid. Debts older than 90 days will be referred to a debt collection agency for recovery.  We have EFTPOS and credit card facilities or can help you to arrange regular automatic or internet payments.  If you envisage fees payment problems you are strongly encouraged to discuss the matter before your consultation with the practice manager whom will endeavour to assist you and work out a suitable payment arrangement.

Important Service Information:
• Urgent appointments are for urgent situations and problems or acute sudden onset of Pain. You may need to speak to a nurse before being given an urgent appointment.  Urgent appointments are for short 5-10 minute durations.  Urgent appointment clinics run for short 1 hour durations in the morning and afternoon during the winter months and on demand at other times of the year. To be able to offer the most efficient service we ask that you respectfully do not bring up matters that can be dealt with at a routine appointment.  The following matters should be discussed at routine appointment only:
Long term/ongoing ‘chronic’ conditions i.e. chronic backache
Long term mental health and depression related issues
Insurance/drivers/immigration/WINZ medicals
Routine BP checks
Tiredness

Repeat prescription requests:
• We deal with an extremely high volume of repeat prescriptions so when leaving a message clearly state your name, phone numbers and the medications required. We need 48 Hours notice for a repeat prescription.  A $15 fee for this service is required when prescription is collected.  If the prescription is faxed to a chemist the charge is $18 to cover extra costs. Forms for you to order repeat prescriptions are available at reception. We would encourage you to use these.

Double bookings i.e. ‘slot in’ appointments:
• the practice operates a strict ‘No double bookings’ policy.  What this means is that we will not squeeze an appointment in to another patients appointment slot. The purpose of this policy is to promote patient and clinician safety and standards and to respect those patients whom have booked their appointments in advance.

Minor Surgery:
• Your doctor will decide the necessity for performing a minor operative procedure following routine appointment.  All minor operative procedures must be pre agreed. receptionists and practice nurses are not able to book an appointments without the doctors consent.

Accidents:
• Accidents are not fully covered by ACC, an additional charge will apply for first and subsequent appointments.  If the initial ACC form is filled elsewhere (hospital, City Drs) please bring ACC forms/paper work with you.

Home Visits:
•  These may be arranged with a practice nurse when necessary.  There is an additional charge for this service.

Practice Nurses:
• Our nurses provide services such as smears, liquid nitrogen, ear syringes etc.  Please make an appointment.  There is a fee to see a nurse unless specific service funding is available.
Immunisations/Vaccinations: 
• Full childhood immunisation services are available. Please contact reception to book an appointment. Childhood immunisation procedures are free.  We run a reminder system for immunizations which are overdue.  Please contact a practice nurse if you are unsure whether your child is up-to-date with their immunisations.
Occupational Health Vaccination:
•Please book an appointment with a nurse to discuss individual requirements.  Vaccinations need to be paid for prior to being ordered.  The cost involved is inclusive of administration. 

Vaccinations For Overseas Travel:
•Not currently offered at this practice.

Influenza:
• This vaccine will prevent epidemic influenza only – not coughs and colds.  It is recommended for all those over 65 and for other people with chronic health problems (e.g. Heart or lung disease, diabetes etc.) who are likely to get complications if they got influenza.  The Government will pay the cost for people over 65 years and over for younger people with certain chronic illnesses provided the vaccine is given before the date specified by Government each year.

Cervical Smears:
• These are available by with a practice nurse, nurse practitioner intern or Dr Jip (an appointment is required). We have a reminder/recall system which can help you keep track of when your next smear is due.  Some women may be eligible for free smears.
