FREE nutritional assessment, follow-up and support for people and their families/whanau with a focus on chronic condition(s). The aim is to empower clients to enable them to self-manage their condition to assist with improving quality of life.
Supportive role with other agencies in screening, health promotion and/or public health projects.
Tui Hancock, Mobile Community Nurse
This service targets people and their whanau who have or are at risk of developing chronic health conditions, particularly diabetes, heart and lung conditions. This is a flexible service that can take place anywhere suitable (home, marae etc). Screening, health promotion/prevention and monitoring of existing conditions are included. The aim is for successful self management and prevention of complications

Central PHO Horowhenua and Otaki hold weekly Cardiac Rehabilitation groups.
The topics covered are Medications, Diet, Stress Management, Understanding Heart Disease, Physical Activity, CPR and Emotions.
If you have a cardiac problem or know someone who does then please come along.
The group is on every Wednesday from 3.00 - 4.00pm at the Horowhenua Health Centre in Liverpool Street Levin.
No appointment necessary, just turn up.

Physical Activity Education program is a free service, you can access this service through your GP or through our other clinical staff. The service is able to visit you at home Marae Community Centers or in clinics.

Foot assessment (including vascular, neuralogical and biomechanical), for the identification, treatment and monitoring of patients managing diabetes and who are at risk of further foot pathology.
Horowhenua Health of Older People Service (HOP Team)
The HOP service aims to provide additional support to General Practice Teams to support older adults living in the community and residing in Aged Residential Care. The HOP team works alongside General Practice Teams to identify older adults who may benefit from early identification and prevention of health concerns, additional assessments, and by defining their health needs and working through health issues with a co-ordinated plan of care.
The service provides:
Patient visits, Joint consultations, Clinical medication reviews, support to General Practice Teams and other health professionals and education sessions.
The service is free to the patient and the health provider, providing the patient is eligible for the service as agreed between the HOP service and the General Practice Team. The HOP service is not an acute service.
Team members include 2 Medical Doctors with a special interest in Health of Older People, a Clinical Pharmacist and 2 Nurse Practitioners (Older Adult scope).
